When buying a home on Cape Cod, finding out whether the septic system passes Title 5 is important—but today it is only part of the wastewater due diligence a buyer should perform.
Cape Cod is in the middle of a major, multi-decade change in how wastewater is handled. Some neighborhoods are already being connected to municipal sewer systems. Other towns are building new wastewater treatment facilities and extending sewer networks in phases. Some areas are expected to continue relying on onsite septic systems but may eventually require advanced nitrogen-reducing technology.
That means a Cape Cod property can have a perfectly functioning conventional septic system today and still be affected by a future sewer connection or wastewater-management requirement.
Barnstable County tells homeowners that the region has entered a multi-decade effort to address wastewater pollution and that, broadly speaking, most Cape Cod homes can eventually expect either a sewer connection or a transition from a conventional septic system to newer treatment technology. The County estimates that many homeowners could encounter one of these changes during the next 20 to 25 years, although the timing depends heavily on the property and town.
For someone evaluating Cape Cod septic systems as part of a home purchase, the question therefore should not simply be, “Does this septic system pass?”
It should also be: What is planned for this property over the next five, ten or twenty years?
Why Wastewater Is Such a Major Issue on Cape Cod
Cape Cod depends heavily on onsite septic systems. Unlike a large metropolitan area where most houses send wastewater through municipal sewers to centralized treatment plants, much of the Cape has historically relied on individual septic systems.
That creates a particular environmental challenge.
A conventional Title 5 septic system is primarily designed to protect public health by treating wastewater onsite. It is not designed to remove most of the nitrogen contained in wastewater.
According to the Cape Cod Commission’s wastewater program, septic systems account for approximately 94% of Cape Cod’s wastewater nitrogen and nearly 80% of the controllable nitrogen entering the region’s waters.
Excess nitrogen eventually reaches coastal ponds, bays and estuaries through groundwater, contributing to degraded water quality, algae growth and ecological changes.
This problem led to the Cape Cod Section 208 Area Wide Water Quality Management Plan, approved by the U.S. Environmental Protection Agency in 2015. The plan established a regional framework under which all 15 Cape Cod municipalities are responsible for implementing wastewater and water-quality solutions.
Those solutions do not necessarily mean sewering every house on Cape Cod.
Instead, the emerging system is a combination of municipal sewers, centralized wastewater treatment, smaller community systems, advanced onsite septic systems and other nitrogen-reduction strategies.
What Is Title 5?
Title 5—formally 310 CMR 15.000—is Massachusetts’ set of regulations governing onsite septic systems.
For a home buyer, one of its most familiar requirements concerns property transfers. Massachusetts generally requires qualifying septic systems to be inspected in connection with a property transfer. Under the state’s rules, inspections normally occur within two years before the transfer, although certain circumstances allow an inspection within six months afterward and some types of transfers are exempt.
Buyers should review the state’s current MassDEP guidance for buying or selling property with a septic system rather than assuming every transaction follows exactly the same rule.
A Title 5 inspection examines the existing system and determines whether it passes, conditionally passes or fails under applicable standards.
What it does not necessarily tell you is whether the town plans to install a sewer on the street several years from now or whether the property falls within an area subject to newer nitrogen-reduction requirements.
That distinction has become increasingly important on Cape Cod.
The 2023 Title 5 Changes Changed the Conversation
On July 7, 2023, MassDEP adopted significant changes designed to address nitrogen pollution in certain Cape Cod watersheds.
The regulations created new Natural Resource Areas within designated Nitrogen Sensitive Areas. These include watersheds and embayments where nitrogen limits have been established to protect water quality.
Under the rules, a municipality generally has two paths.
It can pursue a Watershed Permit, allowing the town to implement a coordinated nitrogen-reduction program over approximately 20 years.
Or, where the applicable regulatory conditions are not addressed through a Watershed Permit, properties within designated Natural Resource Areas can become subject to requirements for advanced nitrogen-reducing onsite wastewater treatment.
Barnstable County’s explanation of the 2023 Title 5 changes explains that a Watershed Permit can combine sewers with alternatives such as innovative/alternative septic systems, permeable reactive barriers, wetland restoration, aquaculture and other approaches.
This is why two homes with conventional septic systems—even two homes in the same town—can have very different wastewater futures.
One may be inside a planned sewer district. Another may eventually need an advanced septic system. A third may remain on its existing system for considerably longer.
A Passing Title 5 Does Not Guarantee the System Can Stay Forever
This is one of the most important points for Cape Cod buyers.
A Title 5 inspection primarily tells you about the status of the existing septic system under today’s applicable requirements. It should not be interpreted as a promise that the house will remain on that system indefinitely.
Barnstable County specifically advises new homeowners that most Cape properties are expected eventually to face either connection to a sewer system or conversion from a conventional septic system to newer treatment technology.
Therefore, when comparing two houses, it may be worth investigating their future wastewater obligations as carefully as their current septic condition.
Cape Cod’s Sewer Expansion Is Already a Major Infrastructure Program
This transition is not hypothetical. Cape Cod towns are investing heavily in treatment facilities, sewer mains, pumping stations and other wastewater infrastructure.
In April 2026 alone, the Cape Cod and Islands Water Protection Fund approved approximately $53.1 million in subsidies for ten wastewater projects in Barnstable, Dennis, Eastham, Mashpee and Yarmouth. The fund reported that it had committed more than $360 million in subsidies since 2018.
The projects are also being developed differently from town to town.
Here is where the major Cape Cod communities stand as of August 2026, based primarily on municipal and regional government sources.
Barnstable: Large-Scale Sewer Expansion Is Already Underway
Barnstable has one of the Cape’s most extensive sewer expansion programs.
The Town is implementing Phase 1 of its Comprehensive Wastewater Management Plan. According to the Town of Barnstable’s sewer expansion program, Phase 1 is expected to add roughly 90 miles of new sewer infrastructure and remove approximately 44% of the nitrogen targeted by the town’s plan.
Multiple projects are progressing within that phase. The Route 28 West project is under construction, while other neighborhoods are moving through design and construction planning. For example, the Town says construction on the Phinney’s Lane Neighborhoods expansion is anticipated from fall 2026 through fall 2029.
What this means for a buyer: Do not assume an existing septic system will remain the property’s long-term wastewater solution. Check the Town’s Phase 1 project maps and ask whether the parcel is in a current or future sewer service area, when connection could be required, and what property-owner costs may apply.
Bourne: Long-Term Wastewater Planning Is Still Being Developed
Bourne has existing sewer infrastructure in certain areas, including Buzzards Bay, but the town is also developing a broader Comprehensive Wastewater Management Plan.
The Town of Bourne’s Comprehensive Wastewater Management Plan is evaluating wastewater needs, technologies, treatment alternatives and a recommended long-term strategy. The planning work includes studies involving the Cape Cod Canal and possible shared wastewater infrastructure associated with Joint Base Cape Cod.
What this means for a buyer: Wastewater conditions can vary considerably by location. A property may already have sewer access, may remain on conventional septic for now, or could ultimately be affected by whatever solution emerges from the town’s broader planning process. Parcel-specific confirmation with the Town is particularly important.
Brewster: Expect a Strong Role for Watershed Planning and Advanced Septic Technology
Brewster’s approach differs from communities currently extending large sewer networks.
The Town is updating its Integrated Water Resource Management Plan to account for the newer Title 5 nitrogen regulations, evolving wastewater technology and its watershed obligations.
The Brewster Integrated Water Resource Management program addresses drinking water, ponds, coastal estuaries, wastewater and stormwater rather than treating sewer construction as the only solution.
Brewster has also been working with Watershed Permits, including Pleasant Bay and Herring River planning. Barnstable County identifies Brewster as one of the pilot towns involved in development of a regional management structure for advanced innovative/alternative septic systems.
What this means for a buyer: In Brewster, the important future question may be less about when a large municipal sewer reaches the street and more about whether the property falls within a watershed where advanced onsite treatment or another nitrogen-reduction strategy becomes necessary.
Chatham: Sewer Expansion Continues in Phases
Chatham is well into implementation of its sewer program.
According to the Town of Chatham sewer construction schedule, the Phase 1C-5 Oyster Pond extension remains under construction in 2026, the Stage Harbor Road extension is substantially complete, and the Phase 1F-1 extension is scheduled to start in fall 2026 and continue into 2028.
Chatham also makes clear that a municipal sewer in the road does not eliminate all property-owner costs. Its property-owner guidance states that owners are responsible for designing and constructing the connection from the building to the town sewer lateral.
What this means for a buyer: Determine whether the property is already connected, lies within a completed service area, or is included in an upcoming phase. If connection is expected, obtain estimates for the private-side work rather than assuming the town’s construction project pays for everything.
Dennis: Phase 1 Construction Is Active
Dennis is actively building the first phase of its wastewater program.
The Town is constructing sewer lines, pump stations and infrastructure associated with its new Water Resource Recovery Facility on Theophilus Smith Road. Current 2026 updates show active sewer work in multiple Phase 1 neighborhoods, and the Town’s first phase includes eight pump stations.
By August 2026, sewer work was continuing on streets including Bittersweet Lane and other Phase 1 areas.
What this means for a buyer: If a Dennis property is within Phase 1, the timing of sewer availability can materially affect decisions about spending money on a major repair or replacement of the existing septic system. Buyers should ask the Board of Health and wastewater program how the town is handling properties awaiting sewer connection.
Eastham: A New Wastewater System Is Moving From Planning Into Construction
Eastham historically relied heavily on onsite systems and explored alternatives to traditional sewering. The Town has now concluded that some municipal sewer infrastructure is necessary.
Eastham voters approved funding in June 2025 for a wastewater treatment facility and collection system. The Town of Eastham wastewater infrastructure program includes a small sewer system and provides maps where property owners can check Phase One and Phase Two areas.
The Town expects construction to proceed over multiple years.
What this means for a buyer: Check the Phase One and Phase Two maps before purchasing. A home that currently operates entirely on septic may be located inside one of Eastham’s initial sewer areas.
Falmouth: Great Pond Sewer Construction and a Larger Wastewater Strategy
Falmouth already has sewered areas and is now undertaking additional major expansion.
The Great Pond Phase 1 Sewer Project moved into construction in 2026 after the Town appropriated $60 million for Phase 1 in 2025. Current construction includes a new sewer lift station, force mains and collection infrastructure.
The broader Teaticket-Acapesket sewer concept is intended to collect wastewater from approximately 1,800 parcels in the Great and Green Pond watersheds, although the current Phase 1 service area is smaller.
Falmouth is simultaneously advancing an ocean outfall project that would discharge tertiary-treated wastewater approximately 2,300 feet into Nantucket Sound and support the town’s long-term wastewater strategy.
What this means for a buyer: Falmouth is a good example of why address-level research matters. A property may be in an existing sewer area, the Great Pond expansion area, another future wastewater area, or a location where advanced onsite treatment remains important. Buyers in a proposed service area should also investigate betterments and private connection expenses.
Harwich: Phase 3 Sewer Construction Is Underway
Harwich is actively progressing through Phase 3 of its sewer program.
The Town of Harwich wastewater project page says Phase 3 is expected to take approximately 2.5 years and serve more than 600 parcels. Construction updates were continuing during summer 2026.
The Town’s Comprehensive Wastewater Management Plan is intended to guide sewer extensions across multiple watersheds over time.
What this means for a buyer: Find out not only whether the property is currently sewered but also which future sewer phase the parcel is assigned to. Harwich provides project and phase information specifically for that purpose.
Mashpee: One of the Cape’s Largest Active Construction Programs
Mashpee is currently constructing Phase 2 of its sewer program.
According to the Town of Mashpee Sewer Project Status page, Phase 2 construction began in December 2025 and is currently expected to be completed in spring 2028, with property connections following MassDEP approval.
Phase 2 is Mashpee’s largest phase so far, covering approximately 1,200 properties and including six pump stations along with treatment-facility upgrades and sewer collection construction.
Future Phases 3 through 5 remain part of an adaptive plan and can be modified as water-quality results and other conditions evolve.
What this means for a buyer: If the property is inside Phase 2, sewer connection is much more concrete than a distant conceptual plan. If it is in a later phase, timing is less certain and buyers should not assume the current map or schedule will never change.
Orleans: An Established Sewer System Continues Expanding
Orleans has already completed its Downtown sewer phase and has continued extending the system.
The Town is now advancing the Phase 3 Lakes and Ponds project. Official 2026 Town Meeting information describes Phase 3 as a current sewer construction project, while the broader wastewater program continues to fund engineering and planning for later phases.
The Orleans sewer connection resources explain that once required to connect, owners must arrange an approved connection design, hire an approved contractor and properly abandon the existing septic system.
What this means for a buyer: Ask which sewer phase the property falls within and whether a notice or connection obligation has already been issued. If sewer connection is imminent, obtain realistic costs for engineering, installation, septic abandonment and any equipment that may be needed.
Provincetown: The Clearest Move Toward Town-Wide Sewer Availability
Provincetown has one of the most definitive wastewater trajectories on Cape Cod.
The Provincetown Board of Health wastewater information states that the sewer expansion approved in 2022 is intended to provide town-wide sewer availability by 2030. The Board of Health’s stated position is that connection to municipal sewer is the preferred long-term wastewater solution once service becomes available.
The town points to its dense development pattern, older onsite systems and increasing flooding risk as reasons conventional onsite systems are not viewed as Provincetown’s long-term solution.
What this means for a buyer: A buyer of an unsewered Provincetown property should treat future sewer availability and connection planning as a significant part of ownership due diligence rather than viewing the current septic system as necessarily permanent.
Sandwich: A Targeted Sewer System, Not Town-Wide Sewering
Sandwich illustrates another model.
Its Comprehensive Water Resource Management Plan proposes a relatively targeted municipal wastewater system serving approximately 1,880 residential, commercial and industrial parcels—about 20% of parcels town-wide.
The plan is explicitly adaptive. Sandwich can adjust implementation based on measured water-quality results, emerging wastewater technologies, regional partnerships and financing opportunities.
What this means for a buyer: Being in Sandwich does not by itself tell you whether municipal sewer service is likely. The property’s location within or outside the designated wastewater service areas is much more informative.
Truro: Plans Are Still Evolving
Truro remains earlier in the planning process than towns such as Barnstable, Dennis or Mashpee.
The Town of Truro wastewater and watershed management program is developing a Comprehensive Watershed Management Plan addressing wastewater and nitrogen management.
Importantly, the Town itself cautions that many of the documents on its wastewater page are part of ongoing projects and may be drafts or preliminary, meaning recommendations can change as planning progresses.
What this means for a buyer: Avoid assuming a draft sewer or advanced-septic concept is a final obligation. Ask the Truro Health Department about the parcel, watershed, current Title 5 requirements and the latest status of the town’s plan.
Wellfleet: A Hybrid Sewer and Advanced-Septic Strategy Is Taking Shape
Wellfleet is actively developing a more comprehensive wastewater program.
As of August 2026, the Town’s wastewater advisory process was working on conceptual designs for a wastewater treatment facility, recharge facility, downtown sewer collection system and a sewer phasing plan. At the same time, the Town is evaluating areas where innovative/alternative septic systems may be used.
Wellfleet is also one of the Barnstable County pilot communities exploring a more coordinated management structure for advanced onsite wastewater systems.
What this means for a buyer: Wellfleet’s future appears likely to involve a mix rather than a single town-wide solution. Downtown or other targeted areas may ultimately receive sewer infrastructure while properties elsewhere could remain onsite with more advanced treatment.
Yarmouth: Major Phase 1 Sewer Infrastructure Is Being Built
Yarmouth is constructing a new Water Resources Recovery Facility and the first phase of its sewer collection system.
The Yarmouth wastewater construction program provides Phase 1 maps and construction information. The first phase focuses on areas associated with Route 28, Old Main Street and South Shore Drive and is intended to address nitrogen reaching Lewis Bay, Parkers River and Bass River.
The broader plan calls for sewer collection to be expanded in multiple phases over time.
What this means for a buyer: Buyers should check the Phase 1 map and future wastewater plans rather than simply asking whether the street is sewered today. A property outside the first phase may still fall within a later expansion area.
What Could All of This Mean Financially for a Cape Cod Homeowner?
There is no single “Cape Cod sewer cost.”
Costs differ by town, phase, property configuration and financing arrangement.
A future homeowner could potentially encounter a municipal betterment or other project-related assessment, engineering expenses, the cost of installing the private sewer service from the house to the municipal connection, plumbing modifications, a grinder pump where required, abandonment of the existing septic system, ongoing sewer user charges, or the expense of installing and maintaining an advanced onsite treatment system.
Not every homeowner will pay every one of these costs.
More importantly, a large municipal construction budget does not automatically mean the town is paying every expense associated with an individual home’s connection.
That is why prospective buyers should obtain property-specific information rather than relying on general construction-cost headlines.
Financial Assistance Is Available—but Second Homes Can Be Treated Differently
Barnstable County operates the Cape Cod AquiFund to help residential property owners finance qualifying wastewater work.
As of August 2026, the program offers betterment loans at various rates for septic repair, replacement, advanced I/A systems and municipal sewer connections.
There is an important distinction for buyers of vacation properties.
The lowest 0% and 2% sewer-connection rates currently require, among other conditions, an owner-occupied, year-round primary residence. Seasonal and rental properties may still qualify for AquiFund financing, but currently only under the program’s higher 4% or 5% rate categories. Eligibility and rates can change.
That makes wastewater costs particularly worth investigating when purchasing a Cape Cod second home.
Should You Avoid a House Because It Has Septic?
Not necessarily. A conventional septic system is still normal for many Cape Cod properties.
Likewise, being in a future sewer area is not automatically negative. Municipal sewer infrastructure is being built primarily to improve long-term water quality, and sewer availability can eliminate the need to maintain an individual septic system once a property is connected.
The issue for a buyer is uncertainty. A $30,000 hypothetical septic replacement may make little sense if the street is scheduled to receive sewer service shortly afterward. Conversely, assuming sewer will arrive “soon” can be a costly mistake if that property is actually in a later phase that remains years away or subject to adaptive planning.
The objective is to understand the likely path before purchasing.
What Buyers Should Investigate Before Making an Offer
For a Cape Cod property with an onsite wastewater system, a buyer should investigate:
- Obtain the current Title 5 report and determine the system’s age, condition, design capacity and repair history.
- Confirm whether the property is connected to municipal sewer today.
- If not, locate the exact parcel on the town’s wastewater or sewer-phase map.
- Ask whether the property is in a current, approved, proposed or merely conceptual sewer phase.
- Determine whether the property lies within a Natural Resource Nitrogen Sensitive Area using the MassDEP mapping resources referenced by Barnstable County.
- Ask what Watershed Permit applies to the property and whether it changes advanced septic requirements.
- Ask whether sewer connection will eventually be mandatory and what the current estimated timing is.
- Request information about any betterment, assessment, sewer connection fee or other municipal charge associated with the parcel.
- Estimate private-side costs, including engineering, trenching, plumbing, septic abandonment and a grinder pump if one is needed.
- Check whether any wastewater assessment has already been recorded or paid by the current owner.
- If an advanced septic system may be required, ask what technology is currently approved and what ongoing operation, maintenance and monitoring it requires.
- If financing is important, investigate AquiFund eligibility before assuming a particular interest rate—especially for a vacation or rental property.
Because sewer plans are changing rapidly, the town’s Health Department, wastewater department or sewer commission should be considered the authoritative source for the status of a specific parcel.
The Most Important Question Isn’t “Does It Pass Title 5?”
For decades, the standard Cape Cod real-estate question was straightforward:
Does the septic system pass Title 5?
That question is still important. But in today’s Cape Cod market, it is no longer enough. A better set of questions is:
What wastewater system serves this property today, what is the town planning for this neighborhood, and what might the owner eventually be required to pay for or install?
The answer can be completely different in Barnstable than Brewster, in Provincetown than Truro, or even between two neighborhoods within the same town.
Cape Cod’s wastewater transformation will take decades. Municipal sewers will play a major role, but so will advanced septic systems and other nitrogen-reduction technologies.
For buyers, understanding where a particular property fits into that transition is becoming an increasingly important part of real-estate due diligence.
